2008 Sponsor Application
If you are interested in scheduling a date, fill out the form below and return it as soon as possible. If you have a specific date you might wish the Memorial displayed in your community, please indicate the date in the lower portion of the form.
We try to have the schedule compiled by December of the previous year. Dates are assigned to communities and the sponsor is then notified of available dates. If dates are acceptable, a formal agreement will be sent for a signature confirming those dates. The sponsor will then be requested to send in the deposit. Please, after sending in the form, be patient. When the schedule is completed, all applicants will be notified at that time.
